Volume II of Principles of Practice. The belief that most operational failure happens at handoffs is widely held and poorly sourced. Volume II traces the statistic behind it until it runs out, then follows one business through the same failure at three positions: two functions running incompatible definitions of a qualified lead, a company whose correction to the problem made it worse, and a fulfilment function planning against a number that had been fixed out from under it. The work at a seam is rarely dropped. What matters is what actually has to be shared for a handoff to hold.
